CHS Transcript Request

Clovis High School maintains permanent files of high school transcripts for students who have attended form 1997 to present for both CHS and Choices Alternative School.

A Copy may be requested by following the directions below or by visiting the Guidance Office located at 1900 Thornton.

  1. Print Form
  2. Complete all areas of form
  3. Fax a copy to 575-769-4366 attention registrar or mail to CHS, PO BOX 19000 Clovis, NM 88101

*There is no charge for the transcript.
*Transcript requests are not accepted by phone.
*For records prior to 1997, please visit the Student Support Website and follow the Directions.
*Parents cannot request records for a student if the students is over 18 years of age.
*Transcripts will not be released if a student has an outstanding balance or any type of hold.
*Please include a copy of your picture ID with your request.
